You can do this by calling the customer service number that is located on the back of your insurance card. Any fees not covered by your insurance plan will be your responsibility.)

I do not currently accept insurance plans for NY clients and therefore would be an out-of-network provider. New York clients will be required to self-pay. I will supply you with a superbill to submit to your insurance company. This does not guarentee that your insurance provider will reimbuse you for any fees paid to me. I am sorry for any inconveinence. Please check back often for newly added plans that I accept.
